Treatment Authorization
Before any dental treatment begins, your provider will explain the recommended procedure, discuss alternative options, and answer your questions. Treatment will only be performed with your informed consent.
- You have the right to refuse any recommended treatment.
- You have the right to ask questions about any procedure before agreeing to it.
- You have the right to request a second opinion before proceeding with complex or significant treatment.
- Consent for minors must be provided by a parent or legal guardian.
- Emergency treatment necessary to alleviate pain or prevent serious harm may be performed in accordance with professional standards when a guardian is unavailable, as permitted by applicable law.
Appointment Policy
We strive to see all patients on time and ask that you extend the same courtesy to our team and other patients.
- Please arrive 10 minutes early for your first appointment to complete intake paperwork.
- Cancellations or reschedules require at least 24 hours' notice — cancellations with less notice may be subject to a fee.
- Repeated no-shows or last-minute cancellations may require a deposit for future appointments.
- We understand that emergencies happen. Please call us as soon as possible if an unexpected situation arises — we handle these situations with understanding on a case-by-case basis.
- Late arrivals may need to be rescheduled if the available appointment time is insufficient to complete the planned treatment safely.
Insurance & Payment
Bright Smile Dental accepts most major PPO dental insurance plans. We will file insurance claims on your behalf, but insurance coverage is ultimately between you and your insurance carrier.
- We recommend verifying your coverage prior to your appointment. We are happy to assist with this process.
- Any amount not covered by your insurance (including deductibles, co-payments, and non-covered services) is your responsibility and is due at the time of service.
- Payment is due at the time of service unless a prior arrangement has been made.
- We accept cash, checks, and major credit cards. Payment plan options may be available — please inquire at the front desk.
- Returned checks are subject to a returned check fee.
Records & Radiographs
Your dental records, including X-rays and clinical notes, are maintained as required by applicable state dental board regulations. You have the right to request copies of your records. A reasonable administrative fee may apply for record duplication.
Records transfer to another dental provider may be arranged by submitting a written request and signed release. Original records will not be released, but legible copies will be provided in accordance with applicable regulations.
Photography & Documentation
Clinical photographs or other documentation may be taken for clinical record-keeping purposes. Any use of patient photographs beyond your own clinical record (such as educational purposes or practice marketing) will only be done with your separate, explicit written consent.
Intellectual Property
